Gap free joint replacement in Adelaide: what the HCF program actually means

Out-of-pocket costs are the part of private surgery patients find hardest to plan for. The operation itself gets explained carefully. The bill, often, does not. For HCF members in South Australia there is now a route to hip or knee replacement where that bill is removed almost entirely, and it is worth explaining properly, including the parts it does not cover.
Why a gap exists in the first place
A private joint replacement is not one bill. It is three. The surgeon charges a fee. The anaesthetist charges a fee. The hospital charges for theatre time, the ward stay, the implant and the in-hospital care around it.
Medicare pays a rebate toward the medical fees, and a private health fund pays a further contribution. In most cases those two contributions together fall short of the fee, and the shortfall is the gap. In July 2021 the Federal Government reduced or froze a large number of surgical rebates, which widened that shortfall across the board. This is not specific to any one surgeon. It affects private orthopaedic practice across Australia, and it is set out in more detail on the fees and out-of-pocket costs page.
The result is that a patient with top hospital cover can still face a meaningful out-of-pocket cost for a hip or knee replacement. For some people that is manageable. For others it is the reason surgery gets delayed.
What HCF's No-Gap Joints Program does
HCF runs a national program called No-Gap Joints. Rather than paying a partial rebate and leaving the patient to find the difference, HCF agrees terms in advance with a hospital and with the surgeons and anaesthetists who operate there. Those providers then bill HCF directly at the agreed rate and charge the patient nothing.
It only works when every party is in the program. The fund has to have an agreement with that hospital, the hospital has to be participating, and the surgeon and anaesthetist have to be participating providers. That is why the program runs at a small number of hospitals rather than everywhere.
The clinical care does not change. The implants, the planning, the theatre and the length of stay are the same. What changes is who pays.
Where this sits in South Australia
Eastwood Private Hospital is the only hospital in South Australia participating in the No-Gap Joints Program. Dr Luke Mooney is a participating surgeon there, which means an eligible HCF member having a primary hip or knee replacement with him at Eastwood Private has no out-of-pocket cost for the surgeon, the anaesthetist or the hospital admission.
Eastwood Private opened in August 2025 at 204 Greenhill Road, a few minutes from the Adelaide CBD. It is a $90 million, purpose built facility with six operating theatres, 51 beds and private rooms, and it has physiotherapy, radiology and pathology on site. Dr Mooney's consulting rooms at Orthopaedics 360 are in the same building, so assessment, surgery and review happen in one place. Mako robotic technology is available in theatre there.

Full details of what is covered, who is eligible and how to check your position with HCF.
See the details →What is covered
Under the program at Eastwood Private, HCF covers:
- Hospital accommodation and theatre fees
- Fees from participating surgeons, surgical assistants and anaesthetists
- The agreed prosthesis, meaning the implant itself
- Diagnostic tests carried out during your admission
- In-hospital physiotherapy, from pre-operative education through to post-operative rehabilitation
What is not covered, honestly
Three things sit outside the program, and patients should know about all three before they plan around it.
Your hospital excess. If your HCF policy carries an excess and this is your first hospital admission for the calendar year, that excess is payable to the hospital on admission. It is set by your policy, not by the surgeon or the hospital, and HCF can tell you what yours is.
Outpatient appointments. The program covers a hospital admission. Consultations in the consulting rooms, both the first appointment and the post-operative reviews, are outpatient services. They are charged at the usual consultation fee with a Medicare rebate where a valid GP referral is held.
Care outside the agreed scope. If an unrelated medical condition needs treating during the admission, or a complication requires care beyond what the program covers, costs can arise. This is uncommon, but it is a real exclusion rather than fine print worth glossing over.
Who is eligible
Four conditions have to be met.
- HCF hospital cover that includes hip and knee replacement, held for at least twelve months with all waiting periods served. Overseas Visitors Health Cover is not included
- A primary hip or knee replacement, meaning a first replacement of that joint. Revision surgery and partial knee replacement sit outside the program
- Surgery performed at Eastwood Private Hospital by a participating surgeon
- Clinical criteria met, confirmed by both the surgeon and the hospital
The twelve month requirement is the one people most often fall short on, particularly if they have recently switched funds or upgraded their level of cover to include joint replacement. It is worth checking early rather than at the point of booking a surgical date.
Does gap free mean a compromise?
It is a fair question, and the answer is no. The program changes the billing arrangement, not the operation.
Every hip and knee replacement Dr Mooney performs is planned from a pre-operative CT scan, which is used to build a three dimensional plan of the patient's anatomy before theatre. That plan is delivered with Mako robotic assistance where the hospital has the system, which Eastwood Private does. Hip replacement is performed through the Direct Anterior Approach, a muscle sparing technique. None of that is altered by which fund a patient is with.
The implant is the same, the theatre is the same, and the length of stay is decided on clinical grounds in the same way. Accurate implant position matters because it influences how the joint feels, how it moves and how long it is likely to last. That is determined by the planning and the technique, not by the billing.
Cost should not be the reason to operate
One caution is worth stating plainly. A gap free pathway is a good reason to stop delaying an operation that is already indicated. It is not a reason to have an operation that is not.
Joint replacement is appropriate when arthritis is causing pain and loss of function that non-surgical treatment is no longer controlling, and when imaging and examination support it. Plenty of patients leave a first consultation with a plan that does not involve surgery, and that remains the right answer regardless of what the surgery would have cost.
What to do next
If you are an HCF member with hip or knee arthritis, the practical steps are short. Ask your GP for a referral, which is required for Medicare rebate purposes in any case. Call the rooms on (08) 7077 0157 and mention that you are with HCF, so the team can begin the eligibility check early. Bring any recent imaging to your consultation.
If you proceed to surgery, the rooms verify your cover with HCF and issue written financial consent setting out exactly what you will and will not pay before a date is confirmed. Nobody should be walking into a joint replacement uncertain about the bill.
