What is revision hip surgery?

Revision hip surgery involves replacing or adjusting a hip replacement that is no longer functioning correctly. While most hip replacements last 20 years or more, some require revision earlier — due to wear of the plastic liner, loosening of the components, recurrent dislocation, infection, or fracture around the implant (periprosthetic fracture).

Revision surgery is more complex than primary hip replacement. It requires specialised implants, surgical experience with revision techniques, and careful pre-operative planning. Second opinions are welcome.

Common reasons for revision

Reasons a hip replacement may need revision include:

  • Aseptic loosening (implant loosening without infection)
  • Wear of the bearing surfaces (plastic liner)
  • Recurrent dislocation or instability
  • Deep periprosthetic joint infection
  • Periprosthetic fracture (fracture around the implant)
  • Component fracture or malalignment
  • Adverse reaction to metal debris (ARMD) in metal-on-metal implants

Assessment and planning

Assessment for revision hip surgery involves a careful clinical examination, X-rays, and often additional imaging (CT scan, nuclear medicine scan). Blood tests to check for markers of infection are routine. The pre-operative workup is more involved than for primary surgery, because the cause of failure needs to be identified before the correct revision strategy can be planned.
