We collect it only where you or your referring practitioner choose to provide it, and only to arrange and provide your care. Your ongoing clinical record is held separately by Orthopaedics 360 and is governed by their own privacy policy.
The information we collect depends on which form you use.
Appointment enquiry or contact form: your name and contact details, your preferred consulting location, whether you hold a GP referral, and any description of your condition you choose to provide.
GP referral form (completed by your referring practitioner): the patient’s name, date of birth, sex, address and contact details, Medicare number, private health fund details, WorkCover status where applicable, the presenting complaint and relevant history, and the referring practitioner’s name, practice and contact details.
Patient details form: the information above, together with your occupation, next of kin details, Department of Veterans’ Affairs or concession card details where applicable, your current medications, and your acknowledgement of our fee and transcription terms.
This website may also collect non-identifying technical information such as browser type, pages visited, and referral source for website analytics purposes.
Information submitted through this website is used solely to arrange and provide your care. Form submissions are converted into a document and sent to Dr Mooney’s rooms at Orthopaedics 360, and are used to arrange your appointment and prepare for your consultation. Your information is not used for marketing, is not sold, and is not shared beyond what is necessary to arrange and provide your care.
Patient clinical records are maintained by Orthopaedics 360 in accordance with applicable law and their own privacy policy. Queries about access to clinical records should be directed to the rooms directly.
Information submitted through the forms on this website is transmitted over an encrypted connection (HTTPS) and is stored on servers located in Australia, operated by our web hosting provider on our behalf. It is also sent to Dr Mooney’s rooms at Orthopaedics 360 so your appointment can be arranged.
We take reasonable steps to protect personal information from misuse, interference and loss, and from unauthorised access, modification or disclosure. These steps include encrypted transmission, access limited to those who need the information to arrange and provide your care, and keeping the information only for as long as it is needed for that purpose.
We do not disclose the information you provide through these forms to recipients outside Australia.
